R4-Torsion

History:
  • a pregnant buff presented with uterine torsion.
  • anorexia, constipation.
  • 2 times parturated before.it was her 3rd pregnancy.
Clinical exam:
  • Rectal palpation>>>>>its right sided uterine torsion of more than 180 degree. Cervix is dilated.but lumen obstructed due to torsion.




left uterine ligament is tensed and is on dorsal side while right uterine ligament is on the ventral side as occur in right sided uterine torsion.




Treatment:

Schaffer's method for uterine detorsion.


OBSTETRICAL MANAGEMENT

The animal was casted in the right lateral recumbency with forelimb and hind limb tied separately. A wooden plank was placed over the abdomen. One person was asked to stand over the plank. The animal was rolled in a quick jerk in the same side of torsion and the person over the plank was asked to move over the plank at the same direction of rotation, so that the gravid twisted uterus remains static and the animal moves. The vaginal passage of the animal was examined after each roll to find out whether effective detorsion had occurred through rolling. After three successive rolling complete detorsion was confirmed through per- vaginal examination. The fetus could be palpated through the dilated cervix. The fetus was in the normal anterior longitudinal presentation with slight lateral deviation but was dead.posture was corrected and fetus was pulled with the help of obstetrical chains.
